Primary Options for Acute Care (POAC) is a service available across the Metro Auckland (Tāmaki Makaurau) region — including Te Whatu Ora Te Toka Tumai Auckland, Counties Manukau, and Waitematā. POAC supports healthcare professionals by providing access to a wide range of funded community-based services.

Our primary focus is to ensure patients receive the right care, in the right place, at the right time.

The POAC coordination team works with existing community infrastructure and resources to enable both acute and planned care services, helping to reduce unnecessary hospital visits and support care closer to home.

POAC aims to deliver flexible, timely, and well-coordinated care that meets the unique needs of each patient — often in a location convenient to their home or workplace.
