Online Referral for IV Iron Infusion Services

Use this online referral form to send eligible patients to an approved infusion centre or the patient’s enrolled GP for a funded infusion. Please ensure the patient meets the full clinical eligibility criteria before submitting this referral.

If the patient is eligible for maternity services but does not meet the requirements for publicly funded healthcare, they may either self-fund the infusion (do not use this form) or be referred to hospital for an outpatient infusion.

Where clinical criteria are not met, an iron infusion is not recommended unless an exception has been discussed with the appropriate specialist.

It is essential that patients are fully informed of the potential risks and benefits before proceeding with an infusion.
