Update your POAC e‑claims form
We've moved the POAC advanced forms to a new, more secure server. Most practices have already moved, and we're getting close to switching off the old forms.
If your practice hasn't moved yet, please do this soon so you can keep sending referrals, outcomes and invoices without interruption. A Medtech or MyPractice admin user can do it in about 5–10 minutes.
What's changed
The new forms:
- add extra security on the new server
- include extra fields to support upcoming national reporting requirements
- come with a refreshed POAC menu, with the same links, just easier to navigate
What stays the same
Your existing referrals and patient data are not affected.
Referrals, outcomes and invoicing all work the same way.

What to do
A Medtech or MyPractice admin user needs to update the POAC advanced form. Choose your practice system to see the step-by-step instructions.

Open setup page1Download the form for each district you use
Open the Medtech Evolution setup page: apps.easthealth.co.nz/setup/pms-medtech-evolution.html. Under Primary Options web forms, find your district.
Right-click your district's link and choose Save link as.
Save the file to your desktop or downloads folder. Repeat for Auckland, Counties Manukau and Waitematā.
2Import the form into Medtech
Open Medtech. Click the Medtech icon at the top left, then Options.
In Options, select Advanced Forms then Advanced Forms Manager. Click Action, then Import Form.
Click Browse and find the form you downloaded. Click Validate. When the status shows All OK, click Import.
After a minute Medtech confirms the form has been imported. Click OK, then restart Medtech Evolution. Repeat for each district form.
3Test the form
After restarting, click the Home ribbon, then Search.
Search for a test patient, for example MOUSE MICKEY, and open the record.
In the patient record panel, click Advanced Forms, then New.
In the New Patient Form panel, select the Primary Options form you just imported and click OK.
The new POAC menu opens. The banner colour and buttons may vary by district. Click Test Connection.
You should see SUCCESS for both the communications test and practice identification. Close the form without saving. If you don't see success, contact your Primary Options coordinator.
4Set the invoice account
Click the Medtech logo, then Options > Advanced Forms > Advanced Forms Manager, and click the form you just loaded.
In the View Advanced Form box, select Actions and make sure Create Invoice to Account is ticked. If no account is set, click the … button.
Select the account to invoice and click OK.
Click OK to save. Medtech is now set up for the Primary Options form.
Turning off invoice to account later: first select and delete the account name, then untick Create Invoice to Account, then click OK. If you skip deleting the name, the change may not save.
MyPractice instructions
Open setup page1Download the form for each district you use
Open the MyPractice setup page: apps.easthealth.co.nz/setup/pms-mypractice.html and find your district.
Right-click your district's link and choose Save link as.
Save the file to your desktop or downloads folder. Repeat for Auckland, Counties Manukau and Waitematā.
2Import the form into MyPractice
Open MyPractice. Click Tools > System Setup > Web Form Parameters.
When the URL Editor opens, click Import at the top right.
Select the file you downloaded and click Open.
The new form appears in the URL Editor. To add another district, click Import again. If you have no old forms to remove, click Finish.
3Remove the old form
If the form was installed before, click the grey box to the left of the old form's name and press Delete (some keyboards need Fn + Delete). Click Yes to confirm. When all old forms are removed, click Finish.
4Test the form
Press F2, search for a test patient, select them and click OK.
Select Clinical Notes.
Select the Forms tab.
In the WebForms box, double-click the form you imported.
When the form opens, click Test Connection.
You should see SUCCESS. Click Cancel to close without saving. If you don't see success, contact your Primary Options coordinator.
Using the new POAC form
1Opening the form
Medtech Evolution: open the patient, then Advanced Forms (from the Patient menu or the Patient Record panel). Click New, choose Primary Options [District] POAC and click OK.
MyPractice: open the patient's Clinical Notes, go to the Forms tab and double-click the POAC form under WebForms.
On the new form only the top banner is coloured. If the whole form is coloured, you're still on the old version and need to update.
2What each link does
New Referral starts a new case. Tick each eligibility statement, click Proceed with referral, complete every field marked required, then submit.
Invoice Only adds an invoice to an existing case. Up to 16 service lines can be added.
Outcome + Invoice records the outcome of a case, with the invoice optional.
FULL Referral + Outcome + Invoice does all three in one go.
Update Information adds more detail to an existing case.
Status of Claims shows the current patient's claims. Open Cases lists cases on hold or needing more information, with the coordinator's feedback.
Test Connection checks the link to POAC. Practice identification failures are usually caused by a phone number mismatch: screenshot the result and send it to your POAC coordinator.
3After you submit
Save or OK the form in your PMS once the confirmation pop-up appears.
Don't assume the coordinator team will pick up a new referral automatically. Unless advised otherwise, email or call so they know about the case, and email any referral documents.
If Medtech shows "Unable to invoice no a/c holder has been specified", the invoice account isn't set. Follow Set the invoice account in the Medtech Evolution instructions.
Install all three Auckland district forms
Please install the forms for every district, so you're covered for patients from a district outside your clinic's own.

No action is needed. Your form will transfer to the new server automatically.
