Investigation of DVT
POAC will fund community ultrasound investigation of suspected DVT where:
Patient presents with signs/symptoms of DVT, AND
One of the following:
Pregnant or Superficial Thrombophlebitis or Upper Limb or Repeat scan required within 7-10 days (as advised by haematology/thrombosis), OR
Wells Score >2 (see here), OR
Elevated d-dimer (age-adjusted if 50+ years lab d-dimer)

Important notes:
Delayed ultrasound: Consider anticoagulation (refer Healthpathway)
Bilateral limb ultrasound: Requires haematology endorsement
Exclude Red Flags: If any concerns regarding PE, discuss with Haematology prior to referring to POAC
NOTE: Refer to POAC terms and conditions of use and ensure patient is eligible for public health funding.
Submit Referral
Referrals can be submitted by:
POAC PMS system
Generate a new referral via the PMS POAC form, use the DVT Diagnosis Code. Email radiology request to referral@poac.co.nz
OR
Submit Online Radiology Form here
Once the referral has been received, POAC will coordinate the booking and contact the patient and referrer with booking details.
