Welcome back to Sarah Hyder
We welcome back to the POAC team Sarah Hyder in the role of POAC Regional Nurse Advisor.
Sarah qualified as a registered nurse in 1993 and has held senior nursing positions in Auckland since 2004. These include Clinical Nurse Specialist in Haematology, where Sarah presented research at ISTH, Amsterdam in 2013 and POAC Nurse Leader from 2009-2013.
Sarah has been with Auckland Regional HealthPathways since its inception in 2013, working as the Development Lead. During this time she has led the development and localisation of the pathway content for over 250 pathways, embedding the model across DHBs and PHOs and developing key stakeholder relationships across the metro Auckland region. The programme is now well advanced in its adaptation and localisation of the clinical pathways with 60% of content completed and Sarah has played a pivotal role in achieving this.
Sarah’s clinical interests are in acute care and ensuring the patient journey is safe, closer to home and accessible. Her clinical background and experience will be of true value to the POAC service.
Sarah can be contacted to discuss or advise on complex cases and is also available to provide practice training visits upon request.
